Field Service: Use this report for any work performed for a TMS customer. The customer will receive your report directly, without being reviewed by Tactical Medical Staff. Double check the information twice before sending the report.
Illness or Injury: Use this report to claim any qualifying hours for Illness or Injury. Your direct TMS supervisor must be informed first of your inability to report to work, or finish your shift for medical reason. Provide details such as expected return to work, if you are in isolation, or any other non-confidential information.
Time Sheet Only: Use this report to declare worked hours for TMS, when no client is involved. E.g. shuffling vehicles, taking a vehicle to service facilities, administrative hours outside your scheduled time, etc…
